Oxford Health Plans grants 1st of the month and 15th of the month effective dates.
The only exception is if you are already an Oxford member and you are replacing
an existing employer-sponsored Oxford Group or COBRA plan. In this case,
your new Oxford Individual policy will become effective as of the date of
termination of your employer-sponsored or COBRA plan, thereby resulting in no
lapse of coverage.

To enroll, you must submit the following items:

Application
Oxford Health Plans requires original signatures on all applications.
Photocopies or downloads of the application with original signatures are
acceptable, provided that the copy is clear and legible. Click below to
download the appropriate application:

Payment for the first month's premium.
Oxford requires a check payable to Oxford Health Plans.

Certificate of Creditable Coverage (COCC)
Applicants must provide a Certificate of Creditable Coverage or proof of prior
coverage, if applicable, in order to avoid pre-existing condition penalties.
This can be obtained from your previous insurance carrier.
All paperwork should be sent to

at the address below. For 1st of the month effective dates, paperwork
must be received by the 23rd of the month prior to the requested effective
date; for 15th of the month effective dates, paperwork must be received by the
10th of the month.

ID cards are sent to new members approximately 2 weeks after an application has
been received. In the event that a member requires medical attention
prior to receiving an ID card, a copy of the Individual Application may be used
as proof of insurance.
