In order to apply for Sole Proprietor Insurance in New York, you must meet the following eligibility requirements:
- A Sole Proprietor is defined as the sole owner and only employee of a business. The structure of the business (corporation, LLC, D/B/A, etc.) does not matter. However, if you own a business with a spouse or partner, or if you have any employees, then you are not eligible to apply as a sole proprietor. If you are not eligible for a sole proprietor plan, you may still apply for health insurance as an individual.

Sole Proprietor requires a Business license, form 1040/Schedule C (line 26 is wages) and payroll. Sole Proprietors are eligible if they have at least a $10,000 gross income. Those less than one year old will be accepted on a case by case basis based on business records, invoices, pay stubs and / or contracts they provide.
Rates are the same as for groups.
- Eligible Dependents are defined as a legally married spouse, domestic partner (in HMO or Healthy NY only), or a legally dependent child up to the age of 19 (or to 23 if financially dependent and enrolled as a fulltime student at an accredited educational institution).
